Definition of bichectomy

A bichectomy is an operation to reduce Bichat’s balls. These are fatty structures located on the cheeks, between the cheekbone and the jaw. Depending on the individual, they are more or less developed and give the cheeks their rounded appearance.

Removing these fatty bodies considerably refines the lines of the face. The cheeks are deeper, the cheekbones more prominent and the angles of the jaws better defined. This brings the face closer to today’s beauty standards.

Procedure for a bichectomy

Bichectomy is generally performed by a maxillo-facial surgeon. This professional has a thorough knowledge of facial anatomy and is often trained in the bichectomy technique.

The surgeon will analyse your face, checking that you have made the right choice and that the bichectomy is compatible with your aesthetic wishes. He will also photograph your face and explain the operation in detail.

You muststop smoking completely before and after the operation. Similarly, it is forbidden to take aspirin during the week preceding the removal of the Bichat balls, as this increases the risk of haemorrhage and haematoma.

The bichectomy is a light procedure lasting between 30 and 45 minutes. It is performed on an outpatient basis.

A local anaesthetic is administered to the mouth. The operation can also be performed under general anaesthetic if you prefer.

The surgeon makes a one-centimetre incision inside the cheek, above the upper molars. The scars will therefore be completely invisible, hidden in the mouth. The fatty balls that make up Bichat’s balls are identified and their excess, which forms the rounding of the cheeks, is removed.

This is never a complete removal. The incisions are then stitched with absorbable sutures. You will leave the clinic without a dressing; these sutures will reabsorb on their own.

Precautions to take after a bichectomy

As with any operation, a bichectomy requires you to follow certain rules after the operation to ensure that the healing process runs smoothly.

After the operation, it is advisable to apply ice to the cheeks to reduce swelling. The swelling that appears can be significant for up to 7 days, and this tip limits the discomfort. It is also advisable to sleep with your head elevated and avoid rooms that are too warm.

The pain is not severe; simple painkillers will be enough to make it go away. The surgeon will also prescribe antibiotics to be taken for 5 to 7 days. In any case, recovery is rapid and you can resume your normal activities in 2 or 3 days, unless the swelling is uncomfortable.

For 3 days, patients should eat blended or liquid food. Hot food should be avoided for two days. After each meal, teeth should be brushed with a soft bristle brush. Patients should also rinse their mouths for a fortnight, until they have completely healed. Smoking is also prohibited for a month after the bichectomy.

The result of the operation is visible after about 6 weeks, once the oedema has subsided. Patients with firmer, thicker skin will see the final result after three months.
